Fill out the form below for certificates from CLOSED or SUBSUMED parishes. View list of closed and subsumed parishes, or look at the drop-down box in the form below. The Office of Archives and Records only issues certificates from closed parishes. 

Access to Sacramental Records (Baptism, Marriage, First Communion, Confirmation, etc.)

The Archives will not issue certificates for genealogical purposes. The Archives reserves the right to refuse access to records. 

The following individuals and entities are permitted to obtain sacramental certificates:

  1. The person named in the record, aged 18+.
  2. Current parent or legal guardian of the person named, if the person is under 18PARENTS: If your child is 18+, your child must request the record themselves. 
  3. Church personnel from a diocesan marriage tribunal, diocesan chancellor’s office, diocesan bishop’s office, or a Catholic parish, such as pastor, deacon, or parish secretary. No fee or ID required. Certificate must be mailed directly to the office or parish.
  4. If the person is incapacitated or disabled, the caretaker of the person named. Requestor will need to provide proof of power of attorney and their photo ID with request.
  5. If person(s) is deceased, the child, surviving spouse, parent, or executor of the estate. See required documentation at archstl.org/RECA.
  6. Government or Corporate Agencies (Social Security, U.S. Citizenship Services, insurance companies, etc.). No fee or ID required. Requestor will need to provide an authorization by person (or person’s power of attorney/legal guardian) to release information.
  7. Courts of Law. No fee or ID required. Requestor will need to provide the subpoena or court order.

Payment

Certificates are $10 each and will be mailed within 5-10 business days. Certificates will be sent via regular mail unless otherwise requested. After submitting the form, you will be prompted to pay online by clicking the Submit Online Payment.
