Important Notes:
- A benefit eligible employee must waive or enroll in the health insurance plan.
- NEW! Effective July 1, 2024, the High Deductible Health Plan (HDHP) with Health Savings Account (HSA) will be a third medical plan option, in addition to the Standard and Premier Plans, offered to all benefit eligible employees. However, employees must be eligible for an HSA in order to enroll in this plan. - Eligible employees that choose to enroll in one of the three medical/Rx plans (Standard, Premier, or High Deductible Health Plan with Health Savings Account) are automatically enrolled in the dental and vision insurance through Delta Dental and DeltaVision. You cannot opt out of the dental or vision plan.
- Coverage is available at three tier levels – employee, employee + one eligible dependent, or employee + family. Employee contributions will be deducted from your paycheck on a pre-tax basis.
- To request new or additional ID cards register at http://www.myuhc.com and click on the coverage and benefits tab; there you can print or request mailed ID cards
